Thank you for considering me as your doctor. Whether you suffer from acute post-surgery pain, have a lifelong illness, or anything in between, I truly hope that I can help you achieve significant relief from your pain. I specialize in treating difficult cases that have stymied other physicians, and while no doctor can guarantee success, I am proud to say that I have helped many people reclaim their lives and their health.
Before you come in, you may want to read this short page about my qualifications, philosophy on Pain Management, and the treatments I provide.
At the time of your appointment, please bring a valid photo ID, insurance cards, and co-payment. If you have had an MRI, X-RAY or other imaging study bring the discs and reports if possible. Also, please bring a list of medications and related medical records if you have access to them.
To reduce your wait time before your appointment, our New Patient Coordinator will send you a link to set up your Patient Portal. Please complete the new patient information and sign the consents prior to your appointment. If you are unable to do so, you will be asked to come to the office at least 30 minutes prior to your appointment and you will be able to complete and/or confirm the information using one of our Ipads.
